The contact form asks for your name and email, and if you choose, your phone, what you would like help with, and times that suit you. We use them only to answer you and book a visit.
Health details
Please do not send detailed health information through the form. Records from your visits at the clinic are kept under Ontario's health privacy law, not under this website policy.
Analytics, without cookies
This website counts visits with Cloudflare Web Analytics, without cookies and without identifying you.
Who else sees it
The website is hosted by Cloudflare, a company in the United States, and form emails pass through its email service.
How long it is kept
Form messages are kept only as long as needed to answer you and book your visit.
Your rights
You may ask what is held about you, ask for it to be corrected, or ask for it to be deleted, by writing to hello@example.com. You may also complain to the Office of the Privacy Commissioner of Canada.
